再生不良性貧血 및 急性白血病의 凝血學的硏究

Coagulation Studies on aplastic Anemia and Acute Leukemia.

Abstract


The author carried out coagulation studies in order to elucidate the mechanism of hemorrhage on 61 cases of a plastic anemia and 30 cases of acute leukemia who were seen at Kyungpook National University Hospital over a period of 6 years 7 months between April 1960 and October 1966, as well as on 20 normal subjects serving as control, with the following results.
(A) A plastic anemia
(1) There as marked thrombocytopenia accompanied by decreased capillary resistance, bleeding and recalcification time both prolonged, reduced prothrombin consumption and poor clot retraction.
(2) Generally, fibrin stabilizing factor tended to be decreased, and there was hyperfibrinogenemia accompanied by shortened plasma thrombin time.
(3) An acceleration of plasma clot lysis, and increased fibrinolysis was noted. However, whole blood coagulation time, factor 5, factor 7 and plasma prothrombin were within normal limits.
(4) Factors which were closely related to hemorrhage were considered to be the number of platelets and the fibrin stabilizaing factor.
(B) Acute leukemia
(1) As in aplastic anemia, a marked thrombo cytopenia accompanied by increased capillary fragility, prolonged bleeding time and recalcification time, reduced prothrombin consumption and poor clot retraction was noted.
(2) A mild prolongation of whole blood coagulation time and decreased factor 5 were found, and fibrin stabilizing factor tendeed to be decreased and fibrinolysis was accelerated.
(3) Factor 7, plasma prothrombin, fibrinogen and plasma thrombin were within normal limits.
(4) As in aplastic anemia, factors which were closely related to hemorhage were the platelet counts and the fibrin stabilizing factor.
