Abstract

Portal venous obstruction was classified into twotypes: iatra-hepatic and extrahepatic. In the greatmajority of patients portal venous obstruction occurs within the liver and is usually due to cirrhosis. In the small but interesting extra-hepatic group, the obstruction occurs at any point within the splenic or portal vein. The condition usually gives rise to portal hypertension, with the formation of a collateral circulation between the portal and systemic venous systems. Percutaneous splenoportography was carried out on76 patients. Of these, 8 had a extra-hepatic obstruction without liber cirrhosis.
Radiologically, extra-hepatic portal obstruction maybe divided into three types with differing radiologicalapperances depending on whether the obstruction is inthe splenic vein near the hilum of the spleen, in themain splenic or portal vein. Partial obstruction will be occur in splenic or Portal vein with appearance ofirregular and constant filling defact.
However, blood flow filling defact from superiormesenteric vein near the portal vein on the superior mesenteric and portal junction must be differanciated
Considerable stasis of blood flow in the developed collateral veins were observed. Portal venous pressure through the needle of splenic puncture were over 400mm. H₂O
