Evacuation of the Gravid Uterus by Suction Curettage

Abstract


This study was carried out on 263 patients at the Han-Il Hospital between January 1966 and August 1969. This group comprised 143 cases of artificial interruption of pregnancy, 78 inevital abortion, 24 missed abortion, 13 incomplete abortion, and 5 cases of hydatidiform mole.
Pressure of 30-40 crnHg was obtained by a electric vacuum pump and metal suction curette with a blunt round end of various diameters was used.
In 143 cases of artificial interruption of -pregnancy where the pregnancy was..of.six to thirteen weeks duration, suction evacuation was effective in 96.5% of cases and cervical dilatation was under 1.0-mm dilatation in 96% of cases. After aspiration the uterine cavity was checked with a blunt curette: residua of ovum or decidua were found in 2.5% of cases. Blood loss during operation was proportional to the number of weeks of pregnancy, and lower than with classic method. Injury to the cervix or corpus uteri was not encountered.
Inflammation developed in 2 cases, late postoperative vaginal spotting had persisted in 7 cases.
Menstrual cycles were unchanged in 93.7% of cases. In the cases of early incomplete abortion, inevitable abortion, and hydatidiform mole, suction evacuation was shown to be superior to curettage. In the cases of late incomplete abortion and missed abortions, it was found to be unsatisfactory.
The evidence suggests that the suction evacuation is a satisfactory and atraumatic method´ of inducing abortion, and it is proposed that it should widely replace classic methods used during the first trimester of pregnancy.
