斜骨切斷法에 依한 下顎前突症의 外科的矯正의 一症例

Abstract


Patient S. C. Kim, a 22 year-old Korean soldier, was refered to me because of masticating disturbances, caused by protrusion of the lower jaw and open bite. His health was excellent except for mandibular prognathism. The following treatments were performed:
1) Examination of the patient, radiographs, and study cast model were performed for the analysis of this case.
2) Occlusal adjustments were as detemined by a plaster study casts of the desired postoperative occlusion.
3) The carious teeth were restored with alloy restorations, in addition to a preoperative scalling and prophylaxis.
4) Stout´s multiple loop wiring was used on the mandible, and on the maxilla a Winter´s arch bar was employed for the postoperative intermaxillary fixation.
5) The oblique osteotomy in which the bony cut extended from the mandibular sigmoid notch to the posterior border of the mandibular angle was emplyed for this case.
6) The operating time ranged for 4 hours under local anesthesia.
7) IntermaxiIlary fixation was continued for 6 weeks.
8) The patient was seen three months and five months postoperatively, there was no deviation of the mandible from its predeterminated position, no interference with either sensory or motor innervation and no interfererence with mandibular excursions.
9) The facial appearance and occlusion had been improved very well.
