청소년 집단에서의 자해와 SNS 사용: 체계적 문헌고찰 연구

Abstract
최근 국내외의 자해가 증가하고 있는 추세이며, 특히 청소년들 중에서 자해를 하는 비율이 상당히 높게 보고되고 있다. 연구들에 따르면 온라인 상에서 자해관련 내용에 노출되는 경험은 청소년들의 자해위험을 높이며, 자해를 하는 청소년들은 그렇지 않은 청소년들에 비해 SNS 상에서 더욱 활발한 활동을 하는 것으로 나타났다. 이에 본 연구에서는 청소년 집단에서의 자해와 관련한 SNS 사용 양상을 살펴보고, SNS 사용이 이들의 자해관련 사고와 행동에 미치는 영향을 탐색하고자 체계적 문헌고찰을 통해 선행연구들을 개관하였다. 국내외의 학술 데이터 베이스를 대상으로 2000년 1월 1일부터 2019년 8월 31일까지 학술지에 출판된 경험연구들에 대한 문헌검색을 시행한 결과, 국내에서의 관련연구는 부재하였으며 총 21편의 해외연구(20개의 독립된 연구)가 최종분석 대상으로 선정되었다. 각 연구들은 연구특성 및 연구참여자 특성, 자해와 자살시도력, 자해와 관련한 SNS 사용 및 SNS 사용이 자해에 미치는 영향을 중심으로 분석되었다. 연구 결과, 청소년 자해 집단은 비자해 집단에 비해 SNS를 더 많이 사용하며, 자해행동을 시작하기 전부터 자해나 자살관련 컨텐츠를 검색하는 것으로 나타났다. 자해와 관련한 SNS 사용은 정보나 안내의 획득이나 부정적 정서의 해소, 소속감, 정서적 지지 등의 긍정적 영향을 주기도 하지만, 자해행동의 시작과 유지, 악화에 기여하며 온라인 폭력이나 우울, 섭식문제, 건강하지 않은 성 행동 및 대인관계 등과 관련이 있어 부정적인 영향이 더욱 두드러지는 것으로 나타났다. 이 같은 결과를 바탕으로 향후 청소년 집단에서의 자해와 SNS 사용 간의 관계에 대한 연구 및 개입 방향에 대해 제안하였다.
This study conducted a systematic review of empirical studies on the relationship between self-injury and SNS use among adolescents. Articles published between 01/01/2000 and 08/31/2019 were searched across databases. Twenty-one articles (20 independent studies) were included. Study characteristics, characteristics of participants, history of self-injury and suicide attempts, self-injury related SNS use, and the impact of SNS use on self-injury among adolescents were invetigated. There was no domestic study on the relevant issue and majority of the studies were conducted in western countries. Results showed that adolescents with self-injurious behaviors tend to use SNS more than non-self-injurious peers and search for contents related to self-injury or suicide even before they initiate self-harming. SNS use on self-injury were related to benefits such as acquisition of information or guide, reduction of negative emotions, sense of belonging, and emotional support. But risks of SNS use related to self-injury such as initiation, maintenance and aggravation of self-injurious behaviors, cyberbullying, depression, eating disorders, unsafe sex and interpersonal relationships were more predominant across studies. We discussed implications and limitations of the literature and suggested future directions for research and interventions on self-injury and SNS use among adolescents.
