Abstract

전국 199개 의원급 의료기관에서 2019-2020절기 인플루엔자 의사환자(Influenza-Like Illness, ILI) 감시가 수행되었고, 이들 기관 중 52개 기관에서 수집된 호흡기 환자 검체에 대하여 병원체 감시가 수행되었다. 또한, 200병상 이상 병원급 의료기관 215개소를 대상으로 인플루엔자 입원환자 감시를 실시하였다. 인플루엔자 의사환자 감시와 입원환자 감시의 신고수행률은 각각 98%, 95%였다.
2019-2020절기 인플루엔자 유행주의보 발령 기간은 총 20주(2019.11.15.~2020.3.27.)로 지난 절기 대비 12주 짧았으며, 의사환자 분율은 2019년 52주(2020.12.22.~12.28.)에 49.8명으로 가장 높았으나 지난 절기 73.3명에 비해 낮았고, 이후 지속적으로 감소하여 봄철의 2차 유행은 없었다. 연령군별 의사환자 분율은 7~12세에서 가장 먼저 증가하고 가장 높은 양상을 보였다. 인플루엔자 입원환자 수는 의사환자 분율의 정점보다 2주 뒤인 2020년 2주(2020.1.5.~1.11.)에 정점을 보였다.
2019-2020절기에는 예년에 비해 인플루엔자 유행이 빨리 해제되었는데, 코로나19 유행으로 인한 개인위생 강화 및 사회적 거리두기 등에 의한 것으로 추정된다. 인플루엔자는 매년 겨울에서 봄에 걸쳐 유행하면서 큰 질병부담을 야기하는 질병이므로, 유행을 조기에 인지하고 대규모 확산을 예방하기 위해서는 지속적인 감시체계 운영과 예방접종, 손씻기, 기침예절 등의 예방활동이 필요하다.

Influenza, commonly known as ‘the flu’ is a disease caused by an influenza virus. The aim of this report was to provide the results of the 2019-2020 influenza sentinel surveillance and to make recommendations on controlling and preventing influenza. During the 2019-2020 influenza season in the Republic of Korea (ROK), influenza-like illness (ILI) surveillance was performed at 199 private clinics, and influenza virus laboratory surveillance was performed at 52 of these clinics. In addition, surveillance for confirmed influenza inpatients was conducted in 215 hospitals with more than 200 beds. The reporting rates for ILI and inpatient surveillance were 98% and 95%, respectively.
The ILI consultation rates were above the baseline (5.9/1,000 outpatients) for 20 weeks of the 2019-2020 influenza season (November 15, 2019-March 27, 2020), which was 12 weeks shorter than the previous influenza season. The peak ILI rate of 49.8 (/1,000 outpatients) was reported at week 52 (December 22-December 28, 2019) and was lower than the peak ILI rate of 73.3 (/1,000 outpatients) of the last season. There were no secondary epidemics in the spring of the 2019-2020 influenza season.
The report found that among the 7-12 age group, the epidemic occurred more rapidly and higher than among other age groups. The number of confirmed influenza inpatients was highest at week 2 in 2020 (January 5-January 11, 2020), two weeks after the peak ILI rate.
According to the findings, in the 2019-2020 influenza season, the influenza epidemic decreased earlier than in previous years. Cough etiquette, hand hygiene and social distancing for prevention of the COVID-19 were presumed to be the factors for the lower influenza epidemic than previous seasons. Influenza is prevalent from winter to spring that causes a substantial disease burden on people in Korea every year. In order to recognize the epidemic early and prevent large outbreaks, a continuous surveillance system, vaccination, hand washing, and prevention activities such as cough etiquette are necessary.
