Abstract

결핵은 전 세계 10대 사망원인 중 하나로 질병부담이 큰 감염병이다. 우리나라는 2019년 현재 경제협력개발기구(OECD) 36개 회원국 중 결핵 발생률은 1위, 결핵 사망률은 2위이다.
2010년 이후 국가 결핵관리정책을 적극적으로 추진해 온 결과, 결핵으로 인한 사망자수는 꾸준히 감소하고 있다. 2019년 우리나라의 결핵으로 인한 사망자수는 1,610명(10만 명당 3.1명)으로 전년 대비 10.6% 감소하였고 65세 이상 노인 결핵 사망자수도 1,335명(10만 명당 17.3명)으로 전년 대비 9.9% 감소하였다. 또한 전체 결핵 사망자 중 65세 이상 노인이 차지하는 비율은 ’15년 78.6%, ’17년 82.4%, ’19년 82.9%로 증가하는 경향을 보이고 있다. 노인·노숙인 등 취약계층의 결핵검진을 확대하여 검진 사각지대를 해소 및 결핵 부담을 최소화하기 위해 2020년부터 전국적으로 찾아가는 결핵 검진 사업을 실시하고 치료와 연계하는 정책을 추진하고 있다.

Tuberculosis (TB) is an infectious disease caused by the bacillus Mycobaterium tuberculosis. Worldwide, TB is one of the top ten causes of death and the leading cause of death from a single infectious agent. TB is a preventable and curable disease yet there are millions of new TB cases and TB deaths per year worldwide. In 2018, the first UN high-level meeting on TB was held and a resolution was passed to accelerate efforts to eradicate TB by 2030. The aim of this report was to review the characteristics of TB deaths in 2019 and to analyze TB death trends between 2001 to 2019.
In 2019, the total number of deaths in Korea from TB was 1,610 wherein 143 TB deaths certificates (8.9%) were issued by physicians and filed at local administration offices. Moreover, as a more accurate measure of the number of deaths from TB, the national TB surveillance data from the Korea Disease Control and Prevention Agency (KDCA) confirmed 1,363 deaths from TB (84.7%), and 104 deaths were classified as TB deaths in both the National Health Insurance Service’s (NHIS) data and the National Emergency Medical Center’s (NEMC) emergency records.
Statistics Korea released the Causes of Death Statistics in 2019. TB deaths in 2019 was 1,610 (the mortality rate = 3.1 per 100,000 population), with an reduction of 9.9% from the previous year (2018). Also, 82.9% of the deaths from TB involved elderly patients aged 65 years or older and the TB mortality rate of the elderly was 17.3 per 100,000 population, with an reduction of 9.9% from the previous year (2018).
This report revealed that TB remains a serious health issue among the elderly aged 65 and over in Korea. The implications of this report are reflected in the KDCA’s 2019 policy measure to strengthen TB prevention management and to accelerate the UN’s goal of ending TB by 2030.
