Abstract


The incidence of pelvic fractures with or without dislocation of the sacro-iliac or symphysis pubis, is ~~ontinually increasing. Such injuries not only are serious but can result in disabling complications and death.
The author reviewed 37 cases of unstable pelvic fractures treated at the orthpedic department of kyungpook national university hospital during the period from January 1973 to December 1977, and analyzed as to the cause of injury by mechanical´ forces, type of fracture, average duration for treatment and complications. We have adopted the classific.ttion of unstable pelvis fractures of Dunn and Morris. The fractures were classified as follows: vertical shear, 17 : lateral compression, g :bucket-handle, 7 : pelvic disruption, 4. Twenty-four patients were male and thirteen, female. Traffic., accidents caused the injury in twenty-seven of t:he patients, accident of mine and factory, each three. Two fell from heights. Frquent associated soft tissue injuries were rupture of bladder, 6 patients and rupture of urethra, 10 patients.
The principle of treatment consisted in skeletal traction through the lower limb(distal end of femur) on the involved side and balance skin traction on the normal side. Pelvic sling were used.
In lateral compression type, reduction was accomplished by application of lateral skeletal traction applied through the trochanter on the side of impact. Traction was continued for eight to twelve ir. weeks. After discard the traction physiotherapy start in bed and continued for three to four weeks.
The author conclude that complete reduction of the displacement and an adequate period of immobilization and recumbency is necessary to obtain the best results in treatment of unstable pelvic fractures.
