Abstract
1957년 1월부터 1969년말까지 13년간 서울대학병원 당뇨병관리실에 등록된 1,648명의 당뇨병환자에 대하여 당뇨병성 합병증을 조사연구하여 다음과 같은 성적을 얻었다.
1. 초진당시 하나 이상의 합병증을 가진 환자는 929례(56.4%)이었다.
2. 대사성 합병증으로는 당뇨병 혼수가 12례(0.7%), 백내장 47례(2.9%)이었다.
3. 혈관성 합병증으로는 심 및 대혈관장애 389례(23.6%), 망막증 89례(5.4%), 신증 204례(12.4%)이었다.
4. 당뇨병성 신경증은 479례(29.1%)로서 그 중 204례(12.4%)가 지각장애이었다.
5. 감염을 합병한 증례는 321례(19.5%)로서 그 중 180례(10.9%)가 결핵이었다.
6. 간경변증을 병발한 것은 41례(2.5%)이었다.
7. 당뇨병성 합병증의 빈도는 이병기간이 길면 길수록 높았다. 또한 공복혈당값이 높은 중등증 이상의 증례에서는 비교적 높아지는 경향이 있었다.
Among 1,648 patients diagnosed as diabetes in our diabetic clinic, Seoul National University Hospital for last 13 years (1957^1969), 56.4% had one or more complications. As the train complications, neuropathy was found in 331 (20. 1%) of total patients, retinopathy in 89 (5.4%), nephropathy including albuminuria in 204 (12.4%) and cardiovascular insufficiency including hypertension in 389 (23.6%). There were 321 (19.5%) diabetics complicated with infection, 180 (10.9%) of whom were pulmonary tuberculosis. Ketoacidosis was found in 12 (0. 7 o ), cataract in 47 (2.9%) and liver cirrhosis in 41 (2.5%).
Through more detailed observation on 826 diabetics with complications, we found that these complications had some relationship with patient´s fasting blood sugar level. The duration of diabetes had more significant relationship with the complications; the compications were found in 52.4% of the patients suffering for less than 3 years, 77.4% for 3 to 10 years and 95.5% for over 10 years. it might be conclued that the duration plays an important role in inducing complication to the diabetic patients.
