Abstract


The effects of the first and the repeated dripping of pancuronium were studied in rabbits.
The experiments were performed on 12 rabbits (each group 6 rabbits) of either sex, weighing 1.8∼2.5㎏. The trachea was intubated through tracheostomy under anesthesia with 20% urethane (5ml/㎏) given intraperitoneally. Respiration was controlled by an animal respriation pump. The body temperature was elevated by applying hot jackets on the surface of rabbits.
Stimuli were delivered through a fine silver electrode applied to the common peroneal nerve. The resulting contraction of the muscle activated the force displacement transducer. The output of the transducer was amplified and recorded on a double channel recorder (Biophysiograph, San Ei, Japan).
The degree of neuromuscular blockade following the use of pancuronium, a non-depolarizing agent, was measured with the train of four (TOF) technique (Ail et al., 1975), i.e. the common peroneal nerve was stimulated supramaximally using a short train of four square pulses of 0.2 msec. duration at a frequency of 2㎐ and train duration of 2 sec. Each train was repeated once every 10 seconds.
The ratio of amplitude of the fourth to the amplitude of the first evoked twitch, resulting from transduced anterior tibial muscle contraction. The correlation of the degree of the muscle TOF responses with the time needed to depress and recover the muscle TOF responses were evaluated.
The results were as follows;
1. The time needed to depress the fourth twitch, 25%, 50%, 75%, 90% and 100% of the muscle TOF response was 241, 343, 421, 474, 514 and 548 seconds at the first dripping of Paneuronium and 132, 197, 240, 299, 324 and 352 seconds at the redripping of pancuronium in groupⅠ(38℃) respectively. In group Ⅱ (41℃), the time was 273, 317, 360, 398, 425 and 438 seconds at the first dripping of pancuronium and 173, 223, 265, 327, 365 and 403 seconds at the redripping of pancuronium respectively.
The time needed to depress the TOF response was significantly shorter in the redripping than the first dripping in both group (p<0.05) except in 75% and 100% depression of TOF in group Ⅱ (p>0.05).
2. The time needed to recover the first, the fourth twitch, 25%, 50%, 75%, 90% and 100% of the muscle TOF response was 547, 693, 792, 875, 1,023, 1,245, and 1,740 seconds after the first drpiping of pancuronium and 628, 858, 978, 1,102, 1,262, 1,532 and 2,043 seconds after the redripping of pancuronium in groupⅠ respectively.
In group Ⅱ, the time was 147, 295, 338, 418, 532, 650 and 783 seconds after the first dripping of pancuronium and 177, 320, 402, 481, 585, 713 and 865 seconds after the redripping of pancuronium respectively.
The time needed to recover the muscle TOF response was generally longer after the redripping of pancuronium in both groups, but this result was not significant statisically (p>0.05) except in the recovery of T₁and 25% of the TOF response in groupⅠ(p<0.05).
