Abstract


Ferrokinetic studies, were carried out in 52 cases of blood disorders including aplastic anemia in order to investigate and elucidate their pathogenesis and hematological significance .
1. In hypocellular and normocellular types of a total of 10 cases of aplastic anemia, there was a marked depression of erythropoiesis possiby due to asubstantial decrease of hematopoietic cells and also progressive accumulation of radioiron in both spleen and liver. In hypercellular type, the deficientdelivery of erythrocytes into the circulation resulted from the intramedullary hemolysis with inadeauate compensation.
2. In 6 cases of iron deficiency anemia, ahe underlying mechanism of anemia was considered the hemolysis of efythron with failure of erythropoiesis.
3. The ferrokinetics of 3 cases of pernicious anemia revealed an ineffective erythropoiesis with destruction of marrow erythrons, although the capacity of erythropoiesis was increased fivefold.
4. 10 cases of polycythemia vera (both normal and hemolytic type) was characterized by an increased erythropoiesis.
5. In 7 cases of auto-immune hemolytic anemia, it was inadequate to compensate for a marked reduction of erythrocyte life span though erythropoisis was increased sixfold. the ferrokinetic patters of 2 cases ofhereditary spherocytosis was similiar to that present in auto-immune hemolytic anemia.
6. The ferrokinetics of 3 cases of chronic myelogenous leukemia revealed hemolysis and incomplete compensatory erythropoiesis and incomplete compensatory erythropoiesis. Also extramedullary erythropoiesis was present. 3 cases of myeloid metaplasia showed a minimal marrow uptake while splenic and hepatic erythropoiesis were marked. The mechanism of anemia was similiar th that of chronic mvelogenous leukemia.
7. 1 case of thrombocythemia was shown having hemolysis and extramedullary erythropoiesis. The basic defect of 2 cases of anemia due to infection was relative failure of erythropoiesis and hemolysis.
8. The major changes in ferrokinetics of 5 cases of hemochromatosis were a markedly increased plasma storage iron exchange and a mild hemolysis.
