Abstract

Clinical otosclerosis is a primary focal spongifying disease of the labyrinthine capsule which produces symptoms and signs by fixing the stapes to the oval window by impaction or ankylosis.
A statistical survey of clinical otosclerosis encountered in Seoul National University Hospital from Jan. 1968 to Sept. 1969 was performed.
The results are as followings:
1. There were 12 patients (23 ears) of clinical otosclerosis out of 2,662 ear patients. This means the ratio of 1 case out of 222 ear patients (0.45%).
2. Ratio of the male to female was 1 : 2.
3. Hearing loss appeared in the 11 to 30 years of age in 9 patients (75%) out of 12 patients and 11 cases (91.7%) were bilateral and 1 case was unilateral. Paracusis Willisiana was found in 3 cases (25%).
4. Ear drum findings were normal in 20 ears (80%), slightly turbid in 2 ears (8.7%) and Schwartze sign was found in one ear.
5.Eustachian tubal function was slightly impaired in 2 ears. Mastoid air cells were well pneumatized in 17 ears (74%), moderate in 3 ears (13%), and sclerotic in 3 ears (13%).
6. Celle test was negative in 7 ears (37%) out of 19 ears tested.
7. Average hearing loss of speech range was 44.3 dB in the air conduction test. On the air conduction audiograms flat curves were most commonly observed, 12 ears(52%).
8. Carhart notch was observed in 13 ears (57%) on bone conduction audiogram and average air-bone gap was 37 dB.
