Abstract


So far so-called "Cerebral rete mirabile" or abnormal cerebrovascular network related to the internal carotid arteries has been considered to be particularly common in the Japanese race. This is a report of a case of so-called "Cerebral rete mirabile" in a Korean boy.
A six year old Korean boy was admitted to Department of Neurosurgery, Seoul National University Hospital on May 20, 1969 because of motor weakness in the right extremities. His parents noted that about two years ago their son used to complained of numbness the right lower extremity when he was crying. Soon his right leg became weak and one year later his right arm also became involved. These syinptoms .have .been aggravated when the boy cry or excite and soon recovered to previous state.
On admission no specific neurological abnormality was found except for a mild motor weakness in theright extremities. Bilateral carotid angiograms were performed. Left carotid angiography revealed extremely narrow internal carotid artery and complete obstruction in or at the distal portion of the siphon. In the base of the brain plentiful vascular network was seen and the posterior, anterior and middle cerebral arteries were seemed to be derived from these abnormal vascularnetwork (fig. 1). Right carotid angigoraphy also revealed extremely narrow internal corotid artery and almost complete obstruction in or at the distal portion of the siphon. In the base of the brain plentiful vascular network was also seen. The right anterior cerebral artery was seemed to be derived from the distal portion of carotid siphon but the middle cerebral artery was seemed to be derived from the abnormal vascular network (fig. 2).
These angiographical findings were very similar to that of the so-called "Cerebral rete mirabile" which was described in many Japanese literatures.
