Abstract


Otosclerosis is a localized primary disease of the otic capsule in which new spongy bone causes anklosis of the footplate of stapes, and the white race is more commonly affected than colored.
Recently, surgery of ,partial stapedectomy in stapedial ankylosis and total stapedectomy with wire loop on compressed gelfoam is choice of treatment in hearing loss due to clinical otosclerotic stapes fixation. Selection of cases for surgery in Orient, including korea, Japan, Hongkong and Formosa requires an accurate assessment of the conductive and senseri- neural components a hearing loss of each patient with otosclerosis, as well as a clear comprehension of the diagnosis, pathology and natural history of this unique disease.
Author has performed partial stapedectomy in 8 ears and total stapedectomy in 4 ears for clinical otosclerosisin Korean patient.
The results are as follows:
Under operating microscopic examination (Carl Zeiss, 25X, 40X magnification), tympanic cavity showed slight and moderate whitish plaque deposit on anterior crura and anterior half of the footplate of stapes in 6 ears, on all around the footplate in 3 ears.
However, no pathologic findings were observed in 3 ears. The increased capillaries are running on promontory around the oval window in 8 ears of clinical otosclerosis, but no pathological increased capillary distribution was seen in 4 ears.
At 4 weeks post-operatively, air-bone gag is closed in 83% and no improvement of hearing was observed in 17% by surgery. Histological study revealed osteofibrotic type was most commonly found, but hemangiomatous type, osteoporotic type and chelated type were rare.
