Abstract

The Penicillin allergy has been reported repeatedly by many authors. According to Siegal, the types of penicillin allergy may be differentiated into five groups by clinical signs. Among these, the immediate type has been regarded as the most dangerous manifestation which often results in fatal. Intradermal injection of a very minute dose of penicillin to test the degree of the sensitivity once caused sudden death following typical anaphylactic shock seven years ago in Japan.
The author has made a comparative study on the serological test for the diagnosis of immediate type of penicillin allergy. The results are summarized as follows.
1) Prausnitz-Kuestner reaction seamed to be a most reliable test for the laboratory diagnosis of the possible penicillin shock.
2) Elevation of hemagglutinin titers in patients´ sera or positive skin test may not indicate the immediate type of allergy.
3) The hemagglutinin seemed mostly to be an incomplete antibody to penicillin by the fact that it was blocked by crystaline penicillin, and its titer was elevated when serum was added to the reacting medium.
