Abstract
ÀúÀÚ´Â °ú°Å 9³âµ¿¾È ºÎ»ê´ëÇб³ Àǰú´ëÇÐ º´¸®Çб³½Ç°ú ÀÓ»óº´¸®ÇÐ ±³½Ç¿¡¼ °Ë»çÇÑ 203¿¹ÀÇ °©»ó¼± Á¾¾çÀ» º´¸®Á¶Á÷ÇÐÀûÀ¸·Î ´Ù½Ã °ËÅäÇϰí, WHOºÐ·ù¹ý¿¡ ÀǰÅÇÏ¿© ºÐ·ùÇÏ¿´´Ù. ±×¸®°í Á¾¾çµéÀÇ À¯Çüº° ºóµµ, ³â·Éº° ¹× ¼ºº°ºÐÆ÷ ±×¸®°í º´·Â±â°£À» Á¶»çÇÏ¿´´Ù. ÀúÀÚ´Â ÀÌ Á¶»ç¿¡ ÀÇÇÏ¿© ¾òÀº ¼ºÀûÀ» À§¿¡¼ °í¾ÈÇϰí, ´ÙÀ½°ú °°ÀÌ ¿ä¾àÄÚÀÚ ÇÑ´Ù.
1) ºÎ»êÁö¹æ Çѱ¹ÀεéÀÇ °©»ó¼± Á¾¾ç Áß ¾ç¼ºÁ¾¾çÀÌ 61.1%Àδë, ±×Áß follicular adenoma°¡ ´ëºÎºÐÀÎ 97.6%, ±×¸®°í other°¡ 2.4%¿´´Ù. Follicular adenoma Áß¿¡´Â colloid typeÀÌ 41.9%, simple typeÀÌ 28.2%, fetal typeÀÌ 20.2%, ±×¸®°í embryonal typeÀÌ 7.3%¿´´Ù.
2) ÀÌ Áö¹æ Çѱ¹ÀεéÀÇ °©»ó¼±¾ÏÀº 38.9%Àδë, ±×Áß follicular adenoma°¡ 21.5%, papillary carcinoma°¡ 72.1% undifferentiated carcinoma°¡ 3.8%, squamous cell carcinoma¿Í medullary carcinoma°¡ °¢°¢ 1.3%¿´´Ù.
3) °©»ó¼± ¾ç¼º ¹× ¾Ç¼ºÁ¾¾çÀÇ Æò±Õ³â·ÉÀº °¢°¢ 33.5¼¼¿Í 40.2¼¼¿´°í, ¾Ç¼ºÁ¾¾çÀÌ ¾ç¼ºÁ¾¾ç º¸´Ù ¹ß»ý³â·ÉÀÌ ÀÇÀÇÀÖ°Ô ³ô¾Ò´Ù. ÀÌ Áö¹æ Çѱ¹ÀÎÀÇ °©»ó¼± ¾ç¼º Á¾¾çÀÇ ¹ß»ý³â·ÉÀº ¼¿ïÁö¹æ Çѱ¹Àΰú´Â Â÷À̰¡ ¾ø¾úÀ¸³ª, ¹Ì±¹ÀÎµé º¸´Ù´Â ÀÇÀÇÀÖ°Ô ³·¾Ò´Ù. ¾Ç¼ºÁ¾¾çÀÇ °æ¿ì¿¡ À־ ÀÇÀÇÀÖ´Â Â÷À̰¡ ÀÎÁ¤µÇÁö ¾Æ´ÏÇÏ¿´´Ù.
4) ºÎ»êÁö¹æ Çѱ¹ÀÎÀÇ °©»ó¼± ¾ç¼º ¹× ¾Ç¼ºÁ¾¾çµéÀÇ ³²³àºñ´Â 1:8.3°ú 1:6.4¿´´Ù. ¾ç¼ºÁ¾¾çÀÇ ³²³à ºñ´Â ¹Ì±¹Àο¡ ÀÖ¾î¼ º¸´Ù ºÎ»ê°ú ¼¿ïÁö¹æ Çѱ¹Àο¡¼ ÀÇÀÇÀÖ°Ô ´õ ÀÛ¾Ò°í, ¾Ç¼ºÁ¾¾çÀÇ °æ¿ì¿¡´Â ÀÇÀÇÀÖ´Â Â÷À̰¡ ¾ø¾ú´Ù.
5) ºÎ»êÁö¹æ Çѱ¹ÀÎÀÇ ¾ç¼º ¹× ¾Ç¼ºÁ¾¾çµéÀÇ º´·Â±â°£Àº 3³â ¹Ì¸¸ÀÎ °ÍÀÌ ¾ç¼ºÁ¾¾ç¿¡¼ 60%, ¾Ç¼ºÁ¾¾ç¿¡¼ 70%, ±×¸®°í 10³â ÀÌ»óµÇ´Â °ÍÀÌ ¾ç¼ºÁ¾¾ç¿¡¼ 7%, ¾Ç¼ºÁ¾¾ç¿¡¼ 6%¿´´Ù.
The author have reviewed the 203 cases of thyroid tumors which were studied grossly and microscopic ally during the last 9 years from 1966 to 1974 in our department of pathology and department of clinical pathology, and were kept in the slide file. He classified the cases according to WHO¢¥s histological typing of thyroid tumours and observed percentage distributions of the cases by type, by age and by sex, and duration of symptoms prio to the diagnosis. The results obtained were summarized as follows:
1. The thyroid tumors observed consisted of 61.1 % of adenomas and 38.9% of carcinomas. Of the adenomas follicular adenoma comprised 97.6% and others 2.4%. Follicular adenomas consisted of colloid type(41.9%), simple type (28.2%), fetal type(20.2%) and embryonal type(7.3%).
2. Of the carcinomas follicular carcinoma comprised 21.5%, papillary carcinoma 72. 1%, undifferentiated carcinoma 3.8%, and squamous cell and medullary carcinomas 1.3% respectively.
3. The average ages of patients with adenoma and carcinoma of thyroid were 33.5 and 40.2 year-old, respectively. The age of the patients with carcinoma was significantly older than that of the patients with adenoma. The age of the patients with adenoma in Busan and Seoul areas was older significantly than that of the patients in Boston City, U.S.A., while age of the patient with carcinoma not differ significantly between both.
4. The male and female ratio in the thyroid adenoma was 1:8.3, and that in carcinoma 1:6.4. This ratio in the adenoma was significantly lower in the Korean than in the American, while that in carcinoma not differ significantly between both.
5. Sixty percentage of patients with thyroid adenoma had symptoms for 3 years and under prio to the diagnosis, and seventy percentage of patients with carcinoma had such a history. Seven percentage of adenoma had symptoms for 10 years and above prio to the diagnosis, and six percentage of carcinoma had such a history. In brief, it would seem that adenoma and differentiated carcinoma of thyroid tend to have similar duration and tend to run a similar protracted course.
