Clinical Studies on 116 Cases of Typhoid Fever

Abstract
Clinical studies were carried out on the 116 cases of typhoid fever patients treated in the Busan National University Hospital during last 4 years and 10 months, and the following results were obtained.
1. Seasonal prevalence was similar but slightly prominent in summer. The ratio between male and female was 1:1.2. Most cases (45. 7%) were found in 21-30 age group, and 64.6% of total patients visited the hospital within 2 weeks after the onset of the disease.
2. The most common symptoms were fever(85.3%), chill(74. 1%), headache(64.7%), anorexia (595%), general aching (51.7%) and coughing (41. 4%) in that order of frequency. Important physical signs were abdominal tenderness(75.9%), coated tongue(63.8%) splenomegaly(55.2%), hepatomegaly(37.9%) and relative bradycardia(25. 0%).
3. Peripheral blood findings showed leukopenia in 49. 1% and leukocytosis 3. 4% of the cases.
4. Results of liver function tests were as follows: positive C. C. F. in 4.3%, positive T. T. T. in 14. 7%, elevated G. O. T and G. P. T. in 68.1% and 49. 1% respectively.
5. Widal reaction was positive in 83 patients(71.6%) of 116 and 17 patients of 28 who were 1:80 titer on initial examination showed 1:160 titer after 1 week.
6. Of the 36 patients who received blood culture, Salmonella Typhosa were isolated in 23(63. 9%),
however, of 92 patients who received stool culture, the organisms were isolated in 29 (31. 5%).
7. The complications were found in 26.7% of total cases and the most common complications were intestinal hemorrhage(19. 0%), intestinal perforation(2. 6%), meningitis(1.7%), pneumonia(1.7%), cholecystitis(0.9%) and renal failure(0.9%).
8. Two patients were admitted with complaining of meningeal signs.
9. Clinically favorable response was obtained in the first group treated with 3 Gm CM daily and less favorable in the second group treated 2Gm CM daily. In the first group defeverescence was obtained average in 4. 5 days and the second group in 6 days.
