Abstract


1. Eighty (80) cases of fistula-in-ano seen at the Dept. of Genral Surgery, National Medical Center between period April 1959 to May 1968 are reviewed.
2. Of the 742 cases of anorectal disorders (except congenital malformations of anus and anorectal cancers), the fistula-in-ano cases occupied 10.8% of all.
The ratio of male to femal was 3.7:1. And the most frequent age groups were 3rd and 4th decades which occupied 51.2% of all cases.
3. The fistula-in-ano followed by the anal or perianal absceses which were spontaneously ruptured or incised surgically were the most common etiological bases and accounted for 63.7%.
Tuberculosis accounted for 16.2% and malignancy of anorectum was initiating factor in 3.8%.
4. Predilection site of the fistula -in-ano was posterior part of the anus and multiple lesions were 14 cases. Complete fistula was 78.8% and incomplete fistula was 21.2%.
5. Main subjective symptoms of the cases were anal or perianal discharges, bleedings, pain and constipation in order.
6. Fistulectomy was the most frequent applied surgical intervention and the sphincter muscle was cut vertically as possible without development of incontinence.
The confirmation and complete excision of the primary opening was the most important to prevention of persistant fistula tract and the recurrence of the fistula.
7. The average admission 8 period was 8 days and most of the patients were dischaged within 2 weeks.
