外傷으로 開腹手術을 施行한 41例에 對한 臨床的 考察

Abstract


It was well Known that intra-abdominal injuries follwing non-penetrating or penetrating traumas of the anterior abdominal wall are being encountered with increasing frequency by automobile accidents, beaten up and suicide etc.
clinical observation and evaluation were made on 41 patients who had laparotomy performed for intra-abdominal injuries due to traumas, at Suwon provincial hospital during 30 months from January 1967 to June 1969 and that five urethral injured patients were included in this study for not separated urology in this hospital:
1. 523 emergency surgery had 41 abdominal surgery 7.8%.
2. Ratio between male (34 cases) and female (7 cases) was 4.5:1.
3. The great majority were found in 3rd and 4th decade.
4. The Causes of traumas were Automoebile (21 cases) 19.5%, in working (3 cases) 7.3%, Suicide (3 Cases) 4.9%, Motor Cycle (1 Case) 2.4%, fall down (1 Case) 2.4%.
5. The main clinical symptoms and physical findings were as following orders generalized abdominal parin 29 Cases nausea 25 Cases Vomiting 20 Cases abdominal rigidity 20 Cases Chillnees and fever 13 Cases hematemesis 2 Cases,
6. Among 41 patients, 30 Cases had leucocytosis, more than (10,000/㎣)
7. On plain X-Ray films of the abdomen, 6 Cases were checked in abdominal free air.
8. The Operative findings were as following freqeucy.
Small intestine perforation 11 Cases 27% Spleen rupture 6 Cases 15%, liver injury 5 Cases 12.2% Stomach perforation 4 Cases 9.7% hematoma in retroperitoneum 3 Cases 5% gall bladder rupture 1 Case 2.2% pancreatic pseudocyst 1 Case 2.2% urethral urpture 5 Cases 12.2% not injured 3 Cases 7.3%.
9. The post operative Complications were wound infection. 11 Cases wound disruption 2 Cases intra pelvic abscess 1 Case.
10. The post operative mortality rate. Only 1 Case that diffuse bile peritonitis due to duodenal perforation the mortality rate was 2.4%.
