火傷後 血液像, 血淸主要 電解質 及 尿所見의 變動에 關하여 Studies on the Blood Features , the Serum Electrolyte Concentrations and Urinary Findings in the Burned Patients

Abstract


The WBC, hemoglobin and hematocrit values in blood, and the concentrations of the sodium,
potassium, chloride and CO₂combining power in serum, and urinary specific gravity and the microscopic
urinary RBC were studied in 97 fresh burned patients of 10 to 93% of body surface area. The patients
were admitted between 1 and 7 hours after burning. Venous blood was drawn on 1st, 2nd, 3rd and 5th
days post burn. 24 hours urine were examined on 1st, 2nd, 3rd and 5th days post burn. The results
were studied through clinical and statistical observation and summarized as follows:
1. Generally, there were early and moderate increase in WBC, hemoglobin and hematocrit values
between the first and second days after burning. On the 3rd day they showed gradual decrease and
finally became stable. The decreased hemoglobins have a close relation to the depth of burn.
2. Hyponatoremia and hypochloremia were shown continually from the 1st day after burning.
3. The CO₂combining power value in serum was decreased on the 1st and second days post burn.
4. The concentrations of potassium and calcium in serum were within normal limits on the 1st and
2nd days after burning, but they were gradually decreased as days go by.
5. Urinary specific gravity was high on the 1st day and 2nd day after admission, after that time
on it was gradually decreased and returned to normal limits on 3rd day.
6. Microscopic urinary RBC showed a little increase on the 1st day after burning, and markedly
increased between 2nd and 4th days, and returned to normal limits after the 6th day.
7. The appearance and severity of urinary microscopic RBC were closelt related to the extent and
depth of burn.
