선천성 직장항문 기형증 Congenital Malformation of Anus and Rectum; Clinical Analysis of 54 Cases

Abstract


This is the report of clinical analysis and evaluation of the 54 cases of patient with imper-forate anus, experienced at the surgical department of Severance Hospital, during past 11 years- from Jan. 1964 to Dec. 1974.
The age distribution showed from 1 day old to 22 year old. Male was more commonly aff-ected than female. The ratio of male to female was 1: 7: 1.
The most common type of imperforate anus was type III which was 81,5%.
86.4% of cases, diagnosed by Wangensteen-Rice technique, and 95% of cases, diagnosed by f istulogram were compatible with operative findings.
The most common type of fistula was rectoperineal fistula (26. 5%) and rectourethral fistula (23.5%) in male and rectovaginal fistula (75%) in female.
The, incidence of associated anomaly was 25. 9% and the most common anomaly was anomaly of skeletal system (9.3%).
The preliminary operation was performed in 45.2% of type II A and 77% of type II B. The type of corrective surgery in type IIA was perineal anoplasty (71. 4), and Pull-through oper-ation (28.6%) and in type RIB was pull-through operation (100%).
The corrective surgery was carried out beyond 6 month of age in 25 of 33 cases (75. 8%).
The incidence of complication was 29 2% in colostomised cases and 33.3%in cases of corr-ective operation. The major complications were sepsis and respiratory insufficiency in cases of preliminary colostomy and stenosis and suture line disruption in cases of corrective operation.
The incidence of mortality was 6 of 7 (85.7%) in non-operated cases, 6 of 24 (25%) in cases of preliminary colostomy and 3 of 33 (9.1%) in cases of corrective operation. The ove-rall mortality was 28.3%.
