Abstract


Based on the assessment that the continual stressed positioning applied to the hip causes significant growth anomaly, the hip joint of the young growing rat was continually immobilized in two fored position, i.e., flesion-abduction, and extension, and the statistical, radiological, and histological evaluation of the results were done with the following conclusions.
1. With the measurement of size of capital femoral head, the growth retardation could be recognized form one month of cast immobilization in all two groups and after three months the cange was most pronounced in the extension group.
2. The growth of femur, measured and compared with its maximal length after three months, was accelerated in the extension group, while it was retarded in the flexion-abduction group.
3. With continual immobilization for three months it was noted that in the extension group the flattening of acetabulum and the subluxatio with the increased anteversion of femoral neck developed, whereas in the flexion abduction group the development of acetabulum and the stability of hip were almost within normal limit.
4. Histological study revealed that one month immobilization resulted in the thickening of cartilage mass of the joint, the proliferation of synovial tissue and the deposit of adipose tissue on the acetabular fossa and the irregular, slender trabeculae of acetabular roof could be elicited. The flexion-abduction position of the same period, however, resulted in the minimal thickening of joint cartilage and almost normal development of acetabular roof and trabeculae.
The above experiment has shown that the hip, when forced in the extension positoning immediately following birth, might resume the abnormal growth pattern of acetabular roof and the instability of joint, with deformed growth of the femoral head, while the flexion-abduction position has only insignificant, if any, influence upon them.
