Abstract


The mechanism of renal handling of pyrazinamide an antituberculosis agent, was investigated by means of clearance technique in this study.
Large part of pyrazinamide filtered in the glomeruli is reabsorbed in the tubules, as visualized from the fact that CPZA(clearance of pyrazinamide) amounts only a fraction of simultaneouly measured Ccr(clearance of creatinine).
CPZA-changed linearly with the rate of urine flow, whether it is increased by saline infusion, mannitol infusion or by furosemide, or decreased by clamping ureter catheter during mannitol diuresis. Instead, CPZA remained unchanged until the plasma level of the drug reached 20mg%, and the amount transported in the tubules increased linearly with the increase in the load exhibiting on maximum capacity -for transport.
In no experiment U/P ratio of pyrazinamide lower than unity was observed.
All the evidences indicated that pyrazinamide, like urea, is reabsorbed in all segments of nephron by passive diffusion, and no clue was obtained which might indicate active process or mediation of diffusion, such as facilitated diffusion.
