Abstract


Hirschsprung´s disease in new born infant may manifest itself with the symptom of intestinal obstruction. It may also cause perforation of the gastrointestinal tract.
The early diagnosis and proper treatement are extremely difficult in rapidly deteriorating condition caused by the peritonits due to the perforation secondary to congenital megacolon. Prompt operative intervention is mandatory lest the patient should succumb to the disease.
The authors had an opportunity to treat a two day-old new born male infant who had a perforation of the transverse colon due to Hirschsprung´ s disease.
The perforation was closed without recognizing the true nature of the underlying disease. The infant subsequently developed gradual abdominal distention, intermittent vomiting, malnutrition and diarrhea.
On second opertation, 32 days later, the diagnosis of Hirschsprung´ s disease was made on the findings of constricting rectosigmoid and proximal colonic dilatation.
Sigmoidocolostomy proximal to the constriction was performed but patient expired on the second postoperative day with sepsis. Autopsy confirmed the clinical diagnosis of Hirschsprung´ s disease. Ganglion cells were absent in the Auerbach´ s plexus of a segment of the rectosigmoid distal to or at the constriction.
Brief review of the literature has been attempted.
