Abstract


Author reported two cases of the stab wound of myocardium indusing acute cardiac tamponade. This condition was directly visualized.and j decompressed with surgical approach.
In reviewing literature, many authors have advocated the several procedures of -choice. Recently, this condition has been directed toward the surgical. treatment.
In 1952, Maynard reported his experience in which was. preferred to pericardiotomy and primar3- cardiorrhaphy rather than simple aspiration method in the view point of complications following it. Also, in 1955 Farringer had reviewed and concluded that better results obtained from surgical approach in this field.
In author´s cases, the one was complicated with left hemothorax due to injnry of the intercostal artery and penetrating wound on the periphery of lingular segment of the left upper lobe. The wound of myocardium was located on the right ventricular myocardium just near the descending branch of left coronary artery. But the bleeding was ceased and covered with blood clot. No attempt was made to the wound: All bloody discharge in the pericardial sac was sucked out. The other.´ was characteristically presented the acute cardiac tamponade due to myocardial injury, This case, clinically, was aggravated hour by hour: and roentgenographically revealed the marked englargement of cardiac shadow.
As a result of operation, the wound was located at the apex. of the right ventricular myocardium from which persists continuous bleeding. = Then, cardiorrhaphy was done by two interrupted silk sutures.
The postoperative course of both cases was excellent and discharged at postoperative 14th day, in former, 20th day, the later,
One month later following surgery, no evidence of abnormality was :identified by roentgenographic and electrocardiographic studies, and no differences in the physical activity were noted.
