Left Retrograde Angiocardiographic Studies in the Diagnosis of the Mitral Valvular Diseases

Abstract


The left retrograde angiocardiographic studies using an Amplatz spring coil guide catheter were carried out in 12 dogs for preliminary experiment and in 20 patients with the clinical diagnosis of the mitral valvular diseases.
Following were the summarized results:
1) The mean blood pressures in the dogs were; 170/109 mmHg. in the left ventricle, 165/115 mmHg. in the femoral artery and 170/101 mmHg. in the proximity of aortic valves.
2) The postoperatively confirmed diagnostic accuracies of the physical examination and X-ray studies were approximately 50% and 80% respectively, while those of the left retrograde angiocardiographies were almost 100%.
3) Ventricular ectopic beat and increased ventricular rate were frequently noted in the electrocardiograms duri,ig the angiographic procedures.
4) Following the procedure urinalysis revealed a decrease in 24 hours urinary output and increase of the specific gravity. Mild albuminuria was noted but no glycosuria. Hemograms showed no changes in the erythrocytes, and leucocytes counts. Non-Protein Nitrogen tended to increase in more than 50% of the patients, but blood urea nitrogen showed no changes.
5) The procedure was sometimes accompanied by the side effects like flushing, nausea, general malaise, fever and vomiting but no to such a degree to discontinue.
