Abstract


7 cases of hernifacial spasm were differentiated from functional tic, characterized by persistence in sleep and inability of voluntary inhibition or provocation. Therapeutic block to the affected facial nerve was attempted, originally described by Babcock, utilizing 5 phenol solution 1 to 5 cc. The periocular spasm being predominant, the cervicofacial branch was blocked at the articular tubercle, just above the temporo-mandibular joint. Immediate cessation of the spasm was observed accompanying facial paralysis on the frontal division of the facial nerve in all cases. In spite of disappearance of the periocular spasm, another block ´for remaining spasm around the mouth angle was necessary in most of the patients which was not so forceful as the periocular spasm. 3 to 5 cc. of the phenol solution injected into stylomastoid foramen was able to cease the spasm completely or diminish the intensity and frequency of the spasm around ,the oral angle. The facial paralysis following the block has been improved becoming negligible paralysis later, while the spasm has never been recurred. 4 cases are being followed up, 2 patients still have persisting minimal spasm on the oral angle (less than 5-10% of the original intensity) and in another 2, the spasm disappeared completely in 7 to 19 months after the block.
