Abstract
연구 계획: 증례 보고목적: 내시경을 이용한 추간판 절제술후 발생한 요추의 유착성 지주막염에 대해 성공적으로 치료하였기에 증례를 보고하고자 한다.
선행 연구문헌의 요약: 유착성 지주막염은 감염이나 외상, 수술 등의 의인성 손상에 의해 발생한다고 알려져 왔다.
대상 및 방법: 급성으로 발생한 우측 하지 방사통과 요통을 주소로 내원한 60세 남자 환자로 과거력상 10개월 전 요추 4-5번 추간판 탈출증에 대해 경피적 내시경을 이용한 추간판 절제술을 시행 받은 과거력이 있으며 자기공명검사 소견상 요추 4-5 분절 주변 경막의 섬유성 변화와 마미 신경근들의 유착소견이 관찰되었다.
결과: 스테로이드(steroid pulse therapy)정주 치료 후 증상 호전되었다.
결론: 미세 침습 수술인 내시경을 이용한 추간판 절제술후에도 유착성 지주막염이 발생할 수 있으므로 이에 대한 수술 전 설명 및 이를 예방하기 위한 노력이 필요할 것으로 사료된다.
Study Design: Case report.
Objective: We present a rare case of adhesive arachnoiditis that occurred after percutaneous endoscopic discectomy.
Summary of Literature Review: Adhesive arachnoiditis can be caused by infection, trauma, surgery, and other iatrogenic injuries.
Material and Methods: A 60-year-old man presented with acute onset of lower back pain, radiating to the right leg. His symptoms had developed 10 days previously without any trauma. He had a history of L4-5 percutaneous endoscopic discectomy 10 months ago. There was no evidence of infection in an examination of cerebrospinal fluid obtained from a spinal tap. However, magnetic resonance imaging showed peridural fibrosis, clumping of the nerve roots of the cauda equina with the pia mater, and dural enhancement at the L4-5 level.
Results: His symptoms disappeared after steroid pulse therapy.
Conclusions: Even minimally invasive surgery, such as endoscopic discectomy, can lead to adhesive arachnoiditis of the lumbar spine.
