Abstract


Purpose: To evaluate the availability of direct nasal mucosal ablation and osteotomy using a microdrill in endoscopic dacryocystorhinostomy (DCR).

Methods: We have performed endoscopic DCR in the patients with nasolacrimal duct obstruction at Konyang university hospital from March to September 2003. At first, we located lacrimal sac fossa by moving the probed illuminator through the endoscopic view and injected anesthetics into the corresponding nasal mucosa. Then putting the drill tip on the mucosa directly, we performed nasal mucosal ablation and osteotomy simultaneously. And then one or two sets of silicone tube were inserted. The operation was classified as successful in case of no symptom of epiphora with patency of the lacrimal drainage system.

Results: Five of the 25 patients were male and the remainder female. Operative time was 25~40 minutes and the mean nasal mucosal healing time was 22.75.6 day. The success rate was 88% one month after tube removal. The most common complication was granuloma formation at the osteotomy site.

Conclusions: With the author’s method, we could save operative time in endoscopic DCR and shorten the wound healing time and reduce use of antibiotics to the minimum. Moreover, by virtue of thorough and appropriate postoperative wound care, we could achieve the high success rate.
