Abstract


Burning mouth syndrome (BMS) is a chronic orofacial pain condition characterized as a persistent burning sensation without oral mucosal lesions. Based on recent evidence, BMS is considered as a neuropathic pain condition. Neuropathic change in BMS can occur in various levels of neuraxis, from peripheral to central, and can be confirmed by meticulous psychophysical, neurophysiologic, and neuropatholgical examinations. However, these neuropathic changes are not homogenous in all BMS patients, and peripheral and central neuropathy may present in various combinations in individual BMS patients. Heterogeneity of BMS pathophysiology makes the treatment of BMS more difficult. Therefore, it is essential to identify the underlying neuropathic mechanisms in individual BMS patients. Several clinical information may help to estimate the neuropathic mechanisms. Further studies are needed to investigate and confirm the relationship between clinical characteristics, neuropathic mechanisms, and therapeutic efficacy of medications.
