Surgical Outcome of Cataract Patients Taking Alpha-1 Antagonists

Abstract


Purpose: Alpha-1 (α-1) antagonists, used in benign prostatic hyperplasia, are known to cause complications during cataract surgery such as intraoperative floppy iris syndrome and iris tear. This study is aimed to analyze the complications and prognosis of cataract surgery in the drug takers.

Methods: A retrospective analysis included 67 eyes of 57 patients who were taking α-1 antagonists and underwent cataract surgery. The control group consisted of randomized selection of 22. A comparison was made on the medical records for intraoperative complications, operation time and prognosis.

Results: Posterior capsule rupture was seen in 1 case out of 11 eyes taking terazosin, 1 case out of 9 eyes taking alfuzosin, 5 cases out of 25 eyes taking doxazosin, and 5 cases out of 22 eyes taking tamsulosin. Posterior capsule rupture occurred in 5 eyes out of 22 eyes. There was no statistically significant difference between two groups in complication. 48 out of 67 eyes (71.6%) in patients who took α-1 antagonists and 17 out of 22 eyes (77.5%) in control group showed best corrected visual acuity greater than 0.5. There were no cases of intraoperative floppy iris syndrome or iris prolapsed.

Conclusions: Patients who took α-1 antagonists, and went under cataract surgery did not show intraoperative floppy iris syndrome. There was no statistically significant difference between two groups in postoperative complications.
