Abstract

Ischemic stroke may be accompanied by acute onset thunderclap headache. However, the presence of sentinel headache (headache before neurologic symptoms) has been previously underestimated. We report a case of acute internal carotid artery occlusion in a patient with only acute onset thunderclap headache without any neurologic symptoms who eventually experienced a major ischemic stroke. This case confirms the existence of sentinel headache, and it also shows that occlusion of the internal carotid artery can cause headache without a brain lesion or neurological symptoms.
